Revised templates now available to download

Important updates to Mental Health Treatment Plan templates

The GPMHSC recently published the revised GP MHTP templates. The updated templates are based on the latest available evidence for best-practice mental health assessment and treatment planning in general practice. This includes assessment for trauma-informed care and practice and a recovery-orientated approach to help define patient goals.

The following templates are now available for download (Word and PDF versions):
 

• a template for use with adult patients
• a template for use for child and adolescent patients
• a template as a Subjective Objective Assessment Plan
• a Minimal Requirements template.
